Three major Port State Control regimes—Paris MoU, Tokyo MoU, and India’s DG Shipping—inspected over 52,000 ships in 2025. Despite different geographies and flags, the resulting defect list is remarkably identical.
Why does PSC keep finding the same things year after year?
Here are the key takeaways from our latest deep-dive into the 2025 data:
📷 The Core List Never Changes
Fire safety (fixed installations, alarms, pumps, and dampers) and lifeboats continue to dominate detainable deficiencies globally. Ineffective maintenance and ISM Code non-compliance remain the underlying systemic culprits.
📷 Where the Numbers Are Actually Rising (Per 100 Inspections)
* Structural conditions: +19%
* Health, medical care & social security: +31%
* Propulsion & auxiliary machinery: +13%
(Note: Ballast water spikes were driven by a 2025 joint inspection campaign, not a sudden fleet-wide drop in standards).
📷 The Age Factor in Bulk Carriers
Data from Indian ports highlights a clear risk gradient: 0 out of 48 ships under 10 years old were detained, while detention rates climbed to 10.5% for vessels over 20 years old. Furthermore, non-IACS-classed ships faced a staggering 34% detention rate compared to just 9% for IACS-classed vessels.
📷 Bridging the Readiness Gap
Port State Control counts findings, but it doesn't capture why equipment fails. DG Shipping’s diagnosis points to everyday operational realities: inadequate voyage preparation (especially post-dry-dock), lack of crew familiarity, and shore management that fails to empower Designated Persons Ashore (DPAs).
With Tokyo MoU launching quarterly lists of high- and under-performing companies in 2026, shifting from reactive maintenance to active operational readiness is no longer optional.

Not every ship failure is a glitch. Here's a one-page cyber checklist for navigating officers: at sea, in port, with visitors on board. Free PDF in the article: link in first comment.

Every new strike near the Gulf means crews are sailing through this with the same question: what's next? Seafarers need real-time threat updates and enforceable protection, not just advisories.
#SeafarerSafety#MaritimeSecurity
Hackers got into a system linked to the propulsion of a fully loaded oil tanker.
The VL Prosperity, carrying about 2.3 million barrels, was heading for Galveston when it lost communications.
As a Second Mate, my first question was: what would I have noticed on the bridge?
Reports say no operational problems or danger to the crew, but the investigation continues.
Industry guidance shows cyber problems often look like ordinary equipment faults:
• An ECDIS virus delayed a ship for days, and nobody recognised it as cyber
• A navigation crash left a ship on one radar and paper charts
• A visitor's USB stick brought in malware
My new Officer's Almanac guide for bridge watchkeepers covers what to report, what to avoid and how to fall back safely.

11 Indian seafarers injured in a tanker attack near Hormuz. 🚨
17 of the 19 crew on board were Indian.
As reported by Marine Insight, the Panama-flagged tanker MT On Peace was struck by an unverified projectile near Oman on 6 October.
Twelve crew were injured, 11 of them Indian. Omani authorities helped evacuate them to Khasab, where they are receiving medical treatment.
India's MEA has condemned the attack and called for an immediate halt to strikes on civilian shipping.
If you or your family have someone at sea in the Gulf, keep the emergency contacts handy. I've put them in one place, along with how India is tracking and supporting crew 👇

Stranded in the Gulf, or have a family member on a ship near Hormuz?
Save these emergency contacts now. 🚨
I've put the key emergency contact details in one place, along with how India's DG Shipping is tracking and supporting Indian seafarers in the region.
A dedicated liaison officer is assigned to each affected seafarer.
Please share with your crew, your families and your manning agents.
It could save someone time when it matters most.

When the Hormuz situation escalated, India's DG Shipping directed owners, managers and recruiters to avoid deploying Indian seafarers on Hormuz transits.
As a seafarer, this is what a fast response looks like.
A vessel-by-vessel dashboard now tracks Indian seafarers in the Gulf region whatever the ship's flag, and each affected seafarer gets a dedicated liaison officer.
For those of us at sea, speed and visibility matter as much as the rules themselves.
I wrote about how India is protecting its seafarers in the Hormuz crisis 👇

If you or your family has someone sailing near Hormuz, save this.
DG Shipping India runs a 24x7 control room for seafarers. The emergency contact details are in my latest article, along with the latest hiring advisory for Hormuz voyages.
When a ship is under threat, what matters most is knowing a real person is on the other end of the phone.
What the Ministry reported, with dates and sources in the article:
• Early March: DG Shipping set up a quick response team after the first Indian casualties
• By early April: the control room had handled 4,984 calls and 10,000+ emails
• Repatriation: 286 seafarers by 16 March, over 1,130 by 4 April, and 2,929 in a later update
• July: real-time monitoring of Indian seafarers in the region, plus a hiring advisory for Hormuz voyages
Reports differ slightly in their numbers, so I've dated and sourced each figure.
It was a fast response, and seafarers noticed. If you're sailing that route, or waiting at home for someone who is, the contact list is worth saving.

When a ship is hit near Hormuz, an Indian seafarer's first question isn't about politics.
It's: who do I call, and will someone answer?
I've just published a seafarer's-eye view of India's response to the Hormuz crisis and what officers and ratings should know right now.
What's inside:
• How DG Shipping responded: quick-response team, 24x7 control room, repatriation
• The firmer steps after the attacks: monitoring dashboard and hiring advisory
• Your rights, per the ITF: the right to refuse to sail into high-risk areas
• A "who to reach" section: DG Shipping emergency contacts, escalation levels, and PRANAAM
As of 5 October, the strait remains effectively closed to most traffic, and a tanker was hit on Sunday. Save the numbers before you need them.

Stay safe out there.

Thanks for maintaining this. With the TSS in the center still disrupted, outbound Hormuz transits mean using the temporary Omani-designated routes north or south of it, with UKMTO/JMIC advisories driving the threat picture. A traffic-density layer on those routes would be really useful for watchkeepers.
