The AQUATIC trial may be the most definitive trial of #ESCCOngress
The common combo of ASA and OAC for pts with CAD/stent and AF is clearly been harming people. Lots.
MACE worse, Death worse, Bleeding way worse.
STOP THE ASA when on OAC

🧵 Part 3 — Why ICU RCTs fail (beyond colliders)
1. The puzzle
Decades of critical care RCTs.
Huge effort. Tens of thousands of patients.
Very few reproducible breakthroughs.
This is Part 3 of my series on why ICU trials fail — and why physiology must guide us.
Low vs moderate anticoagulation during VV-ECMO — is a multi-center RCT within reach? Check out this infographic from #SCCMJC to learn more about this study published in @journal_CHEST!

The 2025 AHA/ACC Hypertension Guidelines are out, and the changes are significant. Here are the key takeaways for pharmacists & clinicians:
"Hypertensive Urgency" is now "Severe Hypertension" for BP >180/120 w/o target organ damage.
Move away from IV meds for asymptomatic…
🔍 Abrupt discontinuation of corticosteroids may offer clinical benefits in septic shock patients while minimizing risks associated with prolonged corticosteroid exposure.

💧Fluid creep accounts for more fluid than resuscitation in ICU.
But nobody prescribes it — and most don’t even notice it.
The truth?
🧨 Most ICU patients don’t need routine maintenance fluids.
Here’s what’s really going on 👇🧵
🧂Hyponatremia is a common the ICU, but correcting it smoothly & safely can be challenging.
🧠Rapid correction risks serious complications like ODS/CPM
📈That's why I built NaPathway - a free app - to help clinicians monitor sodium correction

ICU #hemodynamics secrets:
Right ventricular failure is an important cause of insufficient left ventricular preload. In this case, a "fluid bolus" is almost never the right answer

The use of heart-lung interactions in predicting fluid responsiveness (FR) is an old story in Critical Care
In this study of 143 ventilated pts, the authors investigated (i) whether an end-expiratory occlusion test < 15 s & its effects on pulse pressure (PP) reliably detects FR
ICU Snapshots:
A patient underwent exploratory laparotomy for perforated colon. The OR sign-out to us was that he required a few boluses of pressors & 5 liters of ivf over 6 hours but overall looked good and was extubated
These were the pressor boluses intra-operatively:
How long is too long to be performing CPR on patients who suffer in-hospital cardiac arrest? Every patient and situation is obviously different, but it shouldn't come as a surprise that beyond 30 minutes, things don't look too good. 🎩 tip to the authors.

🔥 A rare case of malignant hyperthermia was promptly identified & treated by a pharmacist! A patient with severe PNA requiring cisatracurium developed Tmax 41°C, tachypnea, & respiratory acidosis that failed to respond to external cooling. #pharmessentials#pharmsaves#PharmICU
New SRMA in-press with 6 studies comparing re-arrest rates post-ROSC btwn epi and norepi gtts with NE showing lower re-arrest rates and a non-significant increase is survival to discharge
Should likely be doing NE gtts in most everyone post-ROSC at this point
