Usually its flared within resistant plaque Before you pull (Hard) remember equipment breaks at the weakest point - so deliver a GE as far as possible & then pull both together-
Nightmare in the cath lab 😱 The balloon gets stuck after post-dilation. It gets worse the shaft breaks when you try to pull the system out - you can't retrieve it, you can't snare it. 😨What would you do? @jeroen_sonck @jcspratt @PCRonline
If it is still resistant - pass a second wire & balloon - inflate to free up device - then revert to snare / GE as above

Collateral injury! Epicardial collaterals can have intramyocardial course. RCA CTO. Occluded SVG to RVB-PDA-PLV. Fixed RCA/PLV via ramus epi. Hoping patent skip will supply PDA. Collateral hematoma! This section was intramyocardial, so elected to leave. What would you have done?

Anyone used Astato 40 for Stingray reentry? A real battle with calcium! Calcified vessel not ideal for ADR but retro via septals abandoned. Managed to get Stingray to distal RCA but CP12, Hornet 14 and Astato 20 “bounced off” the calcium. Pulled out Astato 40 which did the trick!
