It can be shocking just how little attention is paid to mobility in hospital. Both at the start: no comment on how the person moves now. Silence on what they were like at baseline. How can we help people “get better” (or home) if we don’t know what better looks like for them?

What is the point of doing routine 4ATs in the ED? Most of us will miss delirium and memory problems on brief reviews if we don't do some specific testing. You might think you're good at just spotting delirium. But you're probably not. (Neither am I)
Today marks 25 years since I stepped through the hospital doors as a qualified nurse. 25 years!! I wasn’t even 25 years old then… It’s been the greatest times, the most challenging times and I have learnt so very many things along the way. But it will always be a privilege ❤️

I hate seeing the words “failed discharge”. This morning it is all over notes where so much time, care, energy and effort was invested. Two words undermine a full MDTs hard work 😓
Is this a good idea? Where is the care plan? Where is the care? What else isnt getting done? This is a list of tasks but no planned care (😬) or RNs managing care it there are few options. Its sad to see care as tasks, its even sadder to see how many people think this is OK.
