News flash. Guideline-based care was supposed to be the basement not the ceiling Oncologists have extensive training/experience. They can make decisions best for pts. If you are an oncologist and you cannot make decisions outside of reading guidelines, maybe time for new job

@ManualOMedicine Classic IHC signature 123-4-56. A histiocytocis (acc to WHO) that's not a histiocytosis (histiopathologists). A diseases that sometimes respond to myeloid- and sometimes to lymphoma-directed tx. And oftentimes to exceedingly costly targeted tx. #BPDCN!
