Congratulations to Dr Chelsea Harris who has just been awarded a University of Otago Māori Doctoral Scholarship to support her PhD!

And again: newer is not better when it comes to medicines. Switching INR –guided warfarin to NOAC in frail older patients with A Fib = more bleeding complications without additional benefit in thromboembolism protection. ahajournals.org/doi/epub/10.11…

Dr Ibrahim Al Busaidi attracted intense interest in his poster presentation #WONCA 2023 @IbzAlBusaidi

Sexual health outcomes for Māori are inequitable. Chelsea Harris @GP_ChCh_UOC presents research-in-progress exploring Māori women's experience of sexual health care: Identifying areas ways to improve clinical care & support Mātauranga (knowledge) to better serve Māori patients

@GP_ChCh_UOC They suggest reconsidering priorities - feels esp important given our overburdened health system + that we "dispassionately inform interested people about the absolute benefits, harms, and burden of screening tests that they consider undertaking." Yes.

"Screening tests with a positive benefit-harm balance measured in incidence and mortality of the target cancer compared with harms and burden may well be worthwhile"

This meta-analysis suggests evidence does not substantiate the claim that common cancer screening tests save lives by extending lifetime, except maybe for CRC screening with sigmoidoscopy. Estimated Lifetime Gained With Cancer Screening Tests jamanetwork.com/journals/jamai…

Chilly but bright winter's day @GP_ChCh_UOC

There was no statistically significant benefit from aspirin in preventing stroke or any conventional stroke etiological subtype in primary prevention in healthy older adults. However, aspirin significantly increased the overall risk of intracranial bleeding jamanetwork.com/journals/jaman…

@RNZCGP conference in Tāmaki Makaurau: slide courtesy of @DeeMangin - thank you for a fantastic, affirming talk & your leadership during Covid. GPs rock 😀

With the plan to increase numbers, how can you justify cutting funding to the Universities that train them?

Shared decision making is not asking what patients want. It's combining what is medically acceptable and what the patient would like/prefer. #GP23

@keeweedoc Could concentrate medical reductionism though....

Brilliant to share research and have a discussion with GPs from across Aotearoa at #GP23. We need to give our GPs the resources and funding to make targeted and safe prescribing (or non-prescribing) decisions that preserve the efficacy of these vital medicines into the future.

Enjoying the update on cefalexin for UTIs by William Guy. Love the feedback from practitioners. "It's likely being used too much. But I don't think it's me..." #GP23

During COVID while the proportion of virtual consults increased, the overall # of encounters decreased with a more marked decrease (50%) for Maori. Pivots to virtual consultations may result in inequity. Interesting poster @IbzAlBusaidi #GP23 @RNZCGP: need to understand the why..

Daphne appears in the tearoom...we're on the run towards spring 😀

Patients' goals & priorities should always be incorporated into medication decisions. New publication shows that the TAPER structured clinical pathway supports patient-centred #deprescribing. @DeeMangin @JournalPatientX journals.sagepub.com/doi/10.1177/23…

Corruption of the NHS in the UK. In 2022, the total value of Big Pharma payments was more than £200m – up 26% compared with 2021. “All drugs have side effects and this drives more sales … It is naive to consider this is in the best interest of the NHS” bit.ly/3PL4A5R

Hemodynamically stable AF pts with H/T, prev DCCV, or HF may warrant active management /ED cardioversion as spontaneous reversion to SR less likely. Pts without these variables can potentially be managed in the community, avoiding hospitalization / cardioversion @IbzAlBusaidi
