My plan for the ER over the next 5 - 10 years is simple 1) Encourage the overproduction of ER physicians to the point they will beg for a shitty job. They won't have a choice in their work environment or pay. 2) Facilitate midlevel creep and make ER physicians compete for work.

Locums model has been great for me. I get ~$70/hr for each hour a doc works. doc works 10 hours I get $700. I have 15 docs working for me = $10.5k. They each do 6 shifts a month = I get $63k for getting them to work a shit pits for me. and they do it gladly. blessed

I love being guilted into thinking I don't deserve to make more money. Not only by the system but by peers. I gave away the majority of my life to save lives. I will not be shamed into believing I deserve less pay while administrators rake in million dollar salaries and bonuses.

healthcare is great. admin can guilt you for not staying late to work while simultaneously having no intention of paying for your time and blaming situation on your inefficiency. that's what I do to my employees. it works esp well with new grads who cannot live without a paycheck

Prediction for US healthcare. Reduce physician pay until the smart ones leave. Replace with low quality, minimally competent physicians who simply want the title and are happy to eat to low wages. Supplement lack of intelligence with AI. Hospital coffers increase exponentially.

What I love about GenZ docs is they DGAF about the American dream or patient care. They accept their mediocre life and work 9 days a month and rely on LLMs like openevidence to practice medicine. "Competence" not "merit" for the win. This is the future we deserve.

If you are a young ER physician your baseline rate should be $350/hr. Don't fall for the benefits as part of your compensation package. These are often overinflated by a LOT. Scam system.

10-15 years ago @teamhealth paid $350/hr to work their shitty ERs. Adjusted for inflation this would be $510/hr. Now they pay less than $300/hr at most sites. Make this make sense when their CEO makes more than $1M a year.

Been a practicing ER physician for 20 years. I have watched unchecked inflation eat take my take home income while watching administrators income rise unchecked. It is time to start saying "NO."
