- local anesthesia: perc right groin access 12F sheath - Penumbra Bolt 12 (right iliac and aorta first) going “against” direction of blood to minimize right leg embolization - next is left iliac, then fempop/tibial with Bolt12 - flow restored bilateral palpable PT and DS DP
Two patients with repeat CT (chest pain episode) within 3 weeks after PE thrombectomy Penumbra (Flash 16). Patient1 submassive extensive bilateral PE on presentation ->complete resolution, Patient2 saddle embolus repeat CT—> tiny remnant thrombus both with resolution of RV strain
Access: - i go Antegrade access for distal fem and pop/tibial. 7F sheah and bolt 7 but rarely i have used 12F antegrade and CAT 12 for well formed resistant thrombus as below (typically symptoms over 2-3 weeks)
