RHC Heather convened a meeting with primary care practices and behavioral health hospitals in Broomfield County so all the providers could identify common gaps, prioritize needs, and share contact information to ensure better continuity of care for patients in Region 16.
Kristen Mast, social worker at St Luke’s Medical Clinic in Larimer County, explains that RHC Jane has changed the way she works. Jane provides connections that make it easier for Kristen’s patients to navigate health & social services, basic social needs, & medical treatments.
RHC @DeeKessler1 and Practice Facilitator Taylor from @CoCHCs explain how they are working to ensure patients have a ride to their doctors’ appointments. They started the SLV Transit Council that includes health care systems, farmers, @ColoradoDOT and more.
At the @SIM_Colorado Collaborative Learning Session,@CHI_Ashlie kicks off the clinic-community connections session. Up to 80% of what impacts our health happens outside the doctors office. @RHConnectors are here to help practices work with local resources who address that 80%.
