Clinical governance frameworks for med spas and wellness clinics. Sold to practices with a medical director. Not legal or medical advice.scope-and-standard.comJoined September 2026
A signed medical director agreement is not a record of oversight.
It settles terms, and the service list belongs in a dated attachment so a menu change does not reopen the contract. The chart review log and the protocol sign-offs are separate records.
Two emergency kits, one checklist on the clipboard.
The sheet says the kit was checked on the 3rd. Which one? Each kit needs its own identifier and its own check record, or a gap in one hides behind the other.
Your carrier asks for last year's near-miss reports.
How many exist? If every report on file describes an event that reached a patient, the form is probably filtering out the ones that never touched a chart.
Which page of your filler protocol names the reporting channel?
If the practice decides an event gets reported, the route should already be printed on the protocol page, not searched for afterward.
An IV ended twenty minutes early. Which line on the record shows it? A record with only a start time cannot say how long anything ran. A stop time, plus a separate field for an early stop with the time and the reason, keeps that answer out of the margin.
When a patient has a reaction mid-treatment, who calls for help and who stays at the chair? If that split is understood but not written down, the first minutes go to sorting it out. Does your emergency plan name both roles by position?
A step in a med spa SOP that reads "the clinic confirms" has nobody performing it. Each step needs one role named against it, and a written line for what happens when that step cannot be completed.
When a medical director leaves, the standing orders they signed stay in the binder. Does anything in the practice mark them as no longer in force? A review date on each order, with the reviewer's name beside it, makes that question answerable.
A staff competency checklist records a verification someone else performed, so the person being checked never fills in their own line. Each validator signature needs a role written beside it, or a later reader cannot tell what it was worth.
Most treatment records name the provider and the area treated. Fewer record which device was used, by serial number or room. If a handpiece is pulled for service next month, could the practice list the patients it was used on?
Two more: let the earliest expiration date in the kit set the next check, rather than a fixed monthly box. And a practice with two kits keeps two records, each with its own kit identifier. The full guide: scope-and-standard.com/guides/med-spa…
On the check side, the initial grid needs an exception column beside it: what was found and the date the correction closed. Without it, a month when the kit was complete and a month when an item was missing look identical.
A med spa emergency kit checklist has two jobs: naming what the kit should hold, and recording that someone opened it and looked. A single grid of initial boxes does the second job badly and the first not at all.
An incident report needs two times: when the event happened and when the report was written. If the form has only one, a note written three days later reads as if it was written that afternoon. Does yours have both?
