@ruveydaiam At least you know your rubberdam isolation was great :)

This is a dens invaginatus which means the enamel folds inwards. When plaque accumulates in this fold, the pulp becomes necrotic. Here I placed an apical plug on vital tissue (yes it's there, I didn't believe it either). Healed one year later. Make endo easier.

What's happening here? Apical radiolucency at a pristine tooth, no trauma, no restoration. Look at the enamel, do you see a pit on the palatinal side and the invagination of the enamel on the x-ray?

@MAbdullah58266 @Mota55em Weird place for the submandibular duct. The opening of that duct in normally located on the lingual side.

Love colours? Watch how the colours in the pulp chamber help you locate mb1 and mb2.

A post-operative factor which positively effects the outcome include the presence of proximal contacts with both mesial and distal adjacant teeth.

The 27 has a massive pulp chamber. It's strange that this tooth is not responding to cold. The pulp of the 26 has obliterated, it make sense this one doesn't respond. When still in doubt, start the endodontic treatment without anesthesia in the 27. Make endo easier.

This patient is in pain. Neither 26 and 27 respond to the cold test. Just by looking at the x-ray, which tooth is more prone to be the culprit?

@DentalSurgnxis Nice video. A rubberdam would complement it.

Pre-operative pain is a predictor for tooth survival. A tooth with pre-operative pain is about 3 times more likely to be lost within the first 2 years after treatment. After that the effect drops significantly.

It's an extra distal root; a radix paramolaris or radix entomolaris. Cool right?! Make endo easier.

Why is this treatment failing? Crown adaptation is good, endo looks nice, no pockets, apical resection done but still... Check out the location of the radiolucency though. Follow the pdl.
