Most agent “memory” setups fail the same three ways: Chat becomes the record. The whole vault gets dumped into context. Tools write files before you agreed. recollect-os is the opposite. Files are the source of truth. Open little — short index, at most two notes. Nothing permanent until you accept. Result: cohesion across sessions and actual persistent memory that stays reviewable. No required embeddings. No vector DB. No background graph product. Honest limit: free-form host edits can still bypass the gate. The durable path is propose → you accept. Local.
