Medical Tuesday. 1997 Balanced Budget Act. This is the dam. Medicare-funded residency slots were frozen. Republican Congress. Clinton signed it. Medical schools later grew. More Americans graduated. The funded seats did not grow with them. Unmatched U.S. graduates and unfilled positions can sit in the same Match year because the money and the slots were locked. Then Congress and the hospital lobby sell visa waivers as the cure. This was likely the plan to begin with. They saw the H1B haul coming in and wanted to go after another top layer of the most profitable occupations. Conrad, recapture bills, and H-1B carve-outs only make sense if you refuse to lift this cap. That refusal is the policy. Lift the cap. Prioritize American graduates.
