People take NAD boosters hoping to age better. A major trial of one of them just delivered a result worth paying attention to. Researchers randomized 410 people with early Parkinson’s disease to nicotinamide riboside (NR) or placebo; 393 were included in the primary analysis. • 500 mg NR twice daily — 1,000 mg total • 52 weeks • Randomized, double-blind, placebo-controlled NR did not improve clinical outcomes. On the main symptom and function scale, the NR group worsened by 2.45 points. The placebo group changed by −0.27 points. The adjusted difference was 2.72 points, favoring placebo. It was statistically significant, but below the 5.2-point threshold cited for clinically important worsening. That’s a disappointing result for NR as a potential Parkinson’s treatment. Does it prove NAD boosters are “mitochondrial poison”? No. It tested oral NR in people with Parkinson’s—not NMN, NAD+ infusions, or healthy adults taking supplements for longevity. But it does challenge the assumption behind a lot of NAD marketing: that increasing a molecule involved in cellular energy will necessarily improve health. For this group, at this dose, over a year, NR didn’t deliver. If you’re buying a longevity supplement, ask what improved in human trials—not just which biomarker went up. ...
