Followup: A percutaneous drain was placed by IR as a marker. Via a 4 cm flank incision between ribs and using a laparoscope to aid visualization, the abscess was drained and stones extracted. Imaging showed complete resolution, and there has been no recurrence in 7+ years.😎

@rbarbosa91 Traditionally, trauma surgery has been felt to be an integral part of general surgery training, and a critical care fellowship really doesn’t provide any general surgery experience. Do you feel that these 1-year fellowship + residency is insufficient for trauma?
@rbarbosa91 Is this really different than say… Hernia surgery?
@rbarbosa91 This unfortunately was a big part of my residency training and a small reason I chose to do thoracic surgery. I also love thoracic surgery and managing thoracic disease.
Trauma surgery is unique among the surgical specialties in that 85% of the daily work could be done by others. It’s that last 15% where the true value of the specialty lies, and it’s the whole point for our existence. Often this isn’t understood, and trauma becomes undervalued.
@ericpaulimd @rbarbosa91 I was gonna say, replace trauma with hernia and it still makes sense
@rbarbosa91 Lot of value in the 85% too... ACS surgeons can usually take out a GB quite a bit better than the average hernia or colorectal surgeon who hasn't taken one out since residency.

@rbarbosa91 True. Took me a while to realize it, but absolutely true. Maybe also true with extremes of other specialties? 85% of general spine can be done by all, but the really hard stuff is where you bring real value
