Can we prevent CAR-T exhaustion before it drives treatment failure? CAR-T therapy has transformed treatment for relapsed/refractory large B-cell lymphoma, but chronic tumor-cell engagement can drive progressive T-cell dysfunction and loss of tumor control. Using single-cell RNA sequencing of tumors at CAR-T progression, investigators identified clonally expanded, proliferating CAR-T cells showing transcriptional features of dysfunction and reduced IL-2 signaling. They then modeled repeated tumor-cell exposure in vitro, finding that chronic rechallenge progressively impaired CAR-T function. A potential solution emerged with pegenzileukin, a non-alpha IL-2 receptor agonist. Rather than simply increasing stimulation, it helped preserve CAR-T expansion and function, improving tumor control in vitro. The findings were even more striking in vivo: CAR-T alone produced only a transient response before tumor progression and CNS relapse, whereas CAR-T + pegenzileukin completely eradicated tumors in the model. The broader lesson: CAR-T failure may not simply reflect too little CAR-T activity—it may reflect sustained antigen engagement pushing the cells into dysfunction. What makes a CAR-T product more effective? In a large analysis of 580 patients with NHL treated with axi-cel, investigators found that the composition of the CAR-T product itself strongly predicted long-term outcomes. The most important feature was a high proportion of naive-like T cells (CCR7⁺CD45RA⁺). Patients with higher levels had significantly longer progression-free survival (HR 0.46, 95% CI 0.31–0.68), without increased toxicity. Importantly, this association was independent of tumor microenvironment characteristics and remained significant after accounting for CAR-T expansion and traditional prognostic factors such as tumor burden and inflammatory markers. The biology was further supported by an inverse relationship between prior lines of therapy and naive-like T-cell abundance, suggesting that earlier treatment may preserve a more favorable T-cell state. The findings were validated in an independent cohort of 203 patients, with mass cytometry confirming that CD8⁺ naive-like T cells lacking exhaustion markers such as PD-1, TIGIT, and LAG-3 were associated with better PFS. The message is increasingly clear: CAR-T efficacy may depend not only on what the CAR targets, but on the cellular state of the T cells carrying it.
