Pathologist are not just reviewing histology slides. We manage the laboratory that makes a physical product. Yes, how the slides/assays are interpreted may change (very few AI offerings for histopathology and many are currently garbage that nobody uses) but we will always be in charge of diagnostics.

@BKRBusinessMin The physician reimbursement for a colon cancer biopsy diagnosis from CMS is less than the cost of fast food for a family of four. People should ask how much they are willing to pay for the correct diagnosis. Yet the Maryland health commission thinks that is still too much.

@DrDiGiorgio The profession fee for a colon cancer diagnosis is approximately $35 from CMS yet the downstream care revenue for cancer care could be hundreds of thousands of dollars. Make it make sense.

If Fit testing is supposed to be done yearly and the Cologuard is done every three years, how are we able to compare them at one time point? Wouldn’t we expect the sensitivity of fit to improve each year. Seems like we are comparing this new test against the worst performance of fit.

Is the improved kappa score referring to improved interobserver agreement for ihc marker agreement, overall tumor classification, or low vs high risk stratification ? The ihc panel identified low risk individuals with a high NPV, and discussed how the relapse group had a higher mortality, but was the panel able to stratify patients by OS?

@PTarantinoMD The current HER2 IHC assays are operating at the wrong dynamic range to distinguish 0 from 1+. This is supported by studies suggesting that accuracy in distinguishing these amongst pathologists is poor. modernpathology.org/article/S0893-…
