Altogether, we argue excessive hyperdirect pathway cortex-STN communication is central to PD and a shared target of dopamine & DBS. This suggests DBS mimics circuit mechanisms of dopaminergic innervation and supports a shift towards neuroprosthetics targeting brain networks. 11/
Structural connectomes showed hyperdirect fibre density was associated with high beta coupling, and functional connectomes showed cortex-indirect pathway fMRI connectivity was associated with low beta coupling, demonstrating the hyperdirect specificity of high beta effects. 10/
Bispectral time delay analysis highlighted cortex-STN interaction via mono- (<10 ms) & poly-synaptic (~30 ms) pathways, with dopamine & DBS suppressing the monosynaptic communication window. This again suggests shared therapeutic modulation of hyperdirect pathway activity. 9/
Directional analysis with multivariate Granger causality revealed that cortex drove information flow with STN. A unique effect of DBS on suppressing high beta cortical drive was also found, which may reflect local effects of DBS on STN vs. widespread action of dopamine in BG. 8/
For network activity, dopamine & DBS suppressed cortex-STN high beta coupling previously linked to hyperdirect pathway, suggesting shared therapeutic modulation of hyperdirect activity. Multivariate coherency analysis recapitulated this and localised coupling to motor regions. 7/
Multivariate analysis (spatio-spectral decomposition) recapitulated these effects, localising high beta power to motor cortex & low beta power to dorsolateral STN. We highlight with a novel 16-contact lead the utility of this method for patient-specific oscillation mapping. 6/
For local activity, we identified the well-described modulation of STN beta power with dopamine & DBS. However, we also found a novel suppression of cortical high beta power with dopamine, showing the importance of considering mesoscale activity in the wider cortex-BG network. 5/
To overcome this, we developed a multimodal approach comparing circuit effects of dopamine & DBS. Externalised ECoG-LFP recordings of cortex & STN were taken at rest in PD patients OFF/ON dopamine & DBS, and combined with normative MRI-based whole-brain connectivity mapping. 4/
Monosynaptic hyperdirect pathway input from cortex to STN is implicated in pathological STN synchrony and is a likely target for DBS effects. However, methodological constraints have hindered investigation of pathway-specific treatment effects of dopamine & DBS in PD. 3/
TL;DR: We demonstrate a shared therapeutic suppression of cortex-STN communication in the high beta band with dopamine & DBS linked to hyperdirect pathway. Distinct effects also seen, with suppression of cortex high beta power with dopamine & high beta cortical drive with DBS. 2/
