This is only the second time in ten years that I’ve sacked off a run because of the weather. The first was in July 2017 when it was about 38 degrees in London. I think it’s reasonable to say I’m not a fair weather runner

A passing runner this morning raised his hand as he approached, so I gave a small wave in return. Only after he passed quite close did I realise he was going for a hi-five & I’d totally snubbed him. Chap on the River Lea about 9am: I apologise for my social ineptitude #ukrunchat

@AlysColeKing This is why we write meaningless phrases in notes like “low risk”, “fleeting thoughts”, “no active intent”, “would tell someone” etc. Instead we must openly acknowledge risk in order to make a plan that best supports. But we need to be working in a culture that allows this

@AlysColeKing Anything but ‘zero suicide’. To do anything about suicide prevention we must first acknowledge suicide as a fact of life. Pretending we can eliminate it just leads to defensive practice and data fiddling, which does nothing to help people who are suicidal.

@AlysColeKing Zero suicide is: A) not actually the goal (for example, medical assistance in dying) B) impossible C) blames caregivers for death in suicide D) implies that all sucides are preventable/mistakes in care which is simply not the case E) leads to stupid policies and positions

@AdrianHarrop Also problems when admitting to wards which are gender-specific. This has improved; I used to have a battle against bed managers claiming things like trans patients couldn’t go to the ward of their gender identity unless they’d had surgery...

@AdrianHarrop As a clinician I see patients being misgendered, pronouns being switched multiple times in the notes, mental state exams commenting “presents as a transgender female” etc, essentially commenting on the degree to which the clinician thinks they ‘pass’.

@AdrianHarrop I think changing culture takes a lot of time and repetition of educational interventions to staff. Diversity training needs to be meaningful, not a tick box. My junior doc and I wear pronoun badges - I hope this can become as widespread as the rainbow lanyard now is
