“Tell me where it hurts.” How back pain radiates can tell you where the lesion is—if you know where to look! Do YOU know where to look? Here’s how to remember the lumbar radicular pain distributions! Keep this cheat sheet as a BACKUP for when you are dealing with BACK pain! ➡️L1 🔸L1 radiates to the groin 🔸Remember that b/c the number 1 is, well, um…phallic. So phallic number 1 radiates to the groin. ➡️L2 🔸L2 radiates to thigh 🔸Two is the number between 1 and 3, so the distribution of L2 is between the distributions of L1 and L3—and between the groin (L1) and knee (L3) is the thigh. ➡️L3 🔸L3 radiates to the knee 🔸Remember L3 is to the knee—easy, it rhymes! ➡️L4 🔸L4 radiates to the calf. 🔸Remember this bc the number 4 looks like the calf, 🔸Top part of the 4 looking like a bulging gastroc & the bottom part of the four is the rest of the calf connecting to the ankle. ➡️L5 🔸L5 radiates to the big toe. 🔸Remember the little rhyme “Five is to the big guy!” 🔸L5 is also foot drop. Remember big guys are heavy, and heavy gravity = drop. ➡️S1 🔸S1 radiates to the side of the foot. 🔸Remember this because both S1 and Side start w/S. So now you know where in the lumbar spine to a look when a patient tells you the pain radiates down their leg! 🚨Remember, there are many variations & this is just a starting guide🚨 But hopefully now remembering the lumbar radicular distributions won’t be a pain in the backside!
🧠 Acute ischemic stroke in 2026: the treatment window is becoming a tissue window The management of acute ischemic stroke has changed dramatically. This useful 2026 review brings together the evolving evidence for thrombolysis, thrombectomy and early antithrombotic therapy. Thrombolysis: tenecteplase is changing practice Alteplase established IV thrombolysis as standard therapy, but tenecteplase has important practical and pharmacological advantages: greater fibrin specificity, a longer half-life and, importantly in emergency stroke workflows, administration as a single IV bolus. The reviewed evidence supports tenecteplase 0.25 mg/kg, maximum 25 mg, and a 2024 meta-analysis found a higher likelihood of excellent functional outcome and less disability at 90 days compared with alteplase. Tenecteplase was FDA-approved for acute ischemic stroke in 2025. But perhaps the bigger conceptual change is that 4.5 hours is no longer an absolute biological boundary. MRI DWI-FLAIR mismatch and CT/MR perfusion can identify selected patients with potentially salvageable tissue beyond conventional time windows. WAKE-UP and EXTEND established this principle, while more recent studies have continued exploring thrombolysis as far as 24 hours in carefully selected patients. Thrombectomy: even a large core does not necessarily mean “too late” Mechanical thrombectomy transformed large-vessel-occlusion stroke, and DAWN and DEFUSE 3 subsequently demonstrated that carefully selected patients can benefit 6-24 hours after onset. Now another boundary is disappearing: the large ischemic core. Multiple randomized trials have demonstrated benefit from thrombectomy in patients previously considered to have too much established infarction. Across these studies, thrombectomy plus medical therapy was associated with better 90-day functional outcomes, with a combined OR of 1.57 (95% CI 1.40-1.76). The review highlights current consideration of thrombectomy for proximal MCA/ICA occlusion with ASPECTS 3-5 and, in selected patients, even ASPECTS 0-2. Posterior circulation treatment has also evolved. For basilar artery occlusion with moderate-to-severe deficits, thrombectomy can be considered up to 24 hours. In contrast, the role of thrombectomy for medium and distal vessel occlusion remains unsettled, with recent trials producing conflicting signals. And reperfusion is only the beginning Recurrent ischemic events cluster early after stroke. Aspirin initiated within 48 hours reduces recurrence and produces a small mortality benefit. For minor ischemic stroke or high-risk TIA, appropriately selected patients benefit from early, short-term dual antiplatelet therapy, rather than simply continuing aspirin alone. Time is brain... Could we be missing one of the most treatable causes of walking and thinking problems in older adults. Normal pressure hydrocephalus (NPH) is a condition where the brain’s fluid spaces enlarge and can disrupt walking, bladder control and thinking. Maroufi and colleagues describe in a new review in JAMA why idiopathic NPH deserves our attention, particularly because it can mimic Parkinson’s disease and Alzheimer’s disease and, importantly, symptoms can improve following treatment. Key points: - Gait problems are frequently the earliest clue, occurring in 85% to 95% of folks, while urinary symptoms and cognitive changes are also common. - Diagnosis requires putting the clinical picture together w/ brain imaging, and removing CSF through a large-volume lumbar puncture can help predict who may respond to a shunt. - Following shunting, gait improves in approximately 70% to 90% of folks, while cognitive and urinary symptoms improve in approximately 50% to 70%. My take: NPH is important because it sits at the intersection of neurology, neurosurgery and aging, and it can look remarkably like Parkinson’s disease, dementia or simply getting older. We should resist prematurely labeling an older person w/ walking, thinking and bladder changes as having an untreatable neurodegenerative disease. NPH is one of those diagnoses we do not want to miss. However, be careful of jumping to a shunt as there can be many complications of shunt therapy. Make sure you have examined the risk-benefit profile for each individual. Here are 5 points that resonated w/ me: 1- Think NPH when gait slowing, falls, cognitive changes and urinary urgency begin clustering together. 2- The classic triad does not need to be complete, and gait trouble may arrive first. 3- MRI findings are important, however imaging alone cannot reliably tell us who will improve following treatment. 4- A large-volume spinal tap and objective measurement of gait before and after CSF removal can provide an important clue about shunt responsiveness. 5- Perhaps the biggest message is simple: recognize NPH early because this is one neurological syndrome where identifying the right folks can potentially restore walking, independence and quality of life. jamanetwork.com/journals/jama/…
Just because it’s called SMALL vessel disease doesn’t mean it doesn’t have a BIG impact! Small vessel disease (SVD) is a BIG contributor to vascular dementia, along w/large vessel cortical infarcts Do YOU know all the faces of small vessel disease? Common imaging markers of SVD include: --White matter hyperintensities (WMHs) --Lacunes --Enlarged perivascular spaces --Microbleeds --Small subcortical infarcts --Brain atrophy But what you see isn’t always what you get! Even in patients w/similar degrees of SVD, clinical symptoms can be very variable. Variability exists because of: --Perilesional penumbra --Remote effects from disruption of brain connectivity --Differing brain reserve & compensatory mechanisms So remember, imaging findings of SMALL vessel disease are just the tip of a very BIG iceberg

Time to go with the flow! Do you know the anatomy of internal carotid artery (ICA)? Do you just say “carotid siphon” & hope no one asks for more detail? Do you even know what "carotid siphon" means?? Here’s how to remember the ICA segments so they just flow off your tongue: C1 = Cervical Number 1 looks like a long straight neck. So C1 travels straight in the neck C2 = Petrous Number 2 makes a curve like a swan’s neck, the same way the petrous ICA curves as it enters the skullbasee C3 = Lacerum Number 3 looks like the zig zag of a ragged cut or laceration, so 3 is a laceration = lacerum C4 = Cavernous ICA takes a turn here, called the genu, which means knee. This is bc it bends like a knee Number 4 looks like a calf with a big gastrocnemius along the back so C4 is at the knee or genu C5 = Clinoid Clinoid comes from the same root as recline What do you do when you reCLINE? You take a break or take 5 Take 5 and reCLINE! C6 = Ophthalmic Round whole in the number 6 looks like an eye, so 6 = ophthalmic C7 = Communicating Number 7 looks an open beak or pac man mouth talking = communicating, so 7 = communicating So now you can be precise in your localization and siphon away the term “carotid siphon”!!
