In 2013, Juan Torres suffered a low-oxygen brain injury which left him profoundly brain damaged.  Doctors told his parents that he was in a permanent vegetative state: unable to speak, control his movements, or follow basic commands, appearing awake but without a glimmer of consciousness. The old dogma that a persistent vegetative state eventually became permanent was based on a 1994 New England Journal of Medicine task force report, which said that an anoxic brain injury became permanent after 3 months and a traumatic brain injury became permanent after 12 months. But we now know that even adults diagnosed as being permanently vegetative can improve.  Thankfully, despite his poor prognosis, Torres’ parents did not give up on him, taking him to a private hyperbaric oxygen clinic and working with him constantly: “Torres’s parents assembled a collection of items they’d use to stimulate him daily: flashcards with ‘colours, words, pictures of the dogs, pictures of family members,’ Perez says. She’d rub sandpaper and feathers along his toes. She’d bring in coffee beans to waft under his nose. ‘I remember that,’ Torres says, as his mother tells the story, ‘the smells, like lemon, mint and hot sauce.’ (They used Tabasco.) Once in a while, family members were sure they saw him respond. “The doctors would say, ‘Yes, but it’s just a reflex,’ ” Perez recalls. “It was difficult to keep hope, because there were so many days when you don’t see any signs.” Torres’ parents even had him evaluated by the famous consciousness researcher, Dr. Adrian Owen. But his tests, including functional MRI scanning, were disappointing: “He came out completely vegetative,” one of the researchers said. “There were no signs of awareness, beyond some reflexive behaviour. It was actually pretty low,” she adds, “even for our vegetative-state patients.” But when the technician called Torres’ family to tell them their son was clinically vegetative, his mom said, “Well, he just started talking.” Because of this stunning news, Torres was brought back to Dr. Owen’s lab for reevaluation. There, he remembered the people who had previously evaluated him, correctly identifying the faces and voices of the people who had thought he was “vegetative.” Torres continued to improve, eventually going to college and back to sports — including wheelchair rugby —  and to composing music on his laptop. If he could have expressed one thought while in hospital, he says it would be this: “Don’t count me out of the fight just because I’m in bed.” @BobbySSchindler @BillAckman @denverhaslam

I remember well seeing some if not most of this live. Evidence that ‘our greatest ally’ didn’t just actively spy on us, but also likely knew about the incoming 9/11 attacks but neglected to tell us. The fact that it got memory-holed like it did tells you all you need to know about who is really in control.

The Journal of the American Medical Association has just published an article on how to gaslight people into accepting “brain death.” It’s pretty condescending in tone, and doesn’t give any good reasons to explain why people in a deep coma on a ventilator — neurologically disabled people — should be redefined as being dead. The only reason given is that these people “fulfill criteria,” but the article doesn’t mention that brain death criteria have never been validated and that even after nearly 60 years there is still no high-quality evidence on the subject of brain death. Brain death is defined simply on the basis of authority, without any evidence, which is a logical fallacy. Here’s what the article says: “Brain death can be difficult for families to understand because patients who fulfill criteria for brain death may demonstrate spontaneous reflexive movements, have a pulse, exhibit chest rise and fall with mechanical ventilation, and have warm skin. If clinicians use nonspecific language when describing brain death, families may experience additional confusion. Brain death may also be complicated for families due to their cultural and religious beliefs or if they experience conflict with health care teams. In addition, concerns about organ procurement and donation may lead to mistrust about the determination of brain death. This article focuses on suggested language for clinicians to use when communicating with families about brain death evaluation and determination.” So, pay no attention to your cultural and religious beliefs or the evidence of your own eyes. Ignore that your loved one is warm, moving at times, and that their heart is beating. Stop thinking for yourself, and let the people who have redefined death do your thinking for you.

How does medical indoctrination work? Why do doctors take so long to question what their authorities have taught them? First, medical students are selected for their outstanding ability to please the teacher: we can memorize large amounts of information and put the “right” answer on the test. We are not chosen for our critical thinking skills, but for our ability to accept what our authorities tell us. Second, we are made to do many uncomfortable things during residency training while stressed and sleep deprived. Taking a bone marrow biopsy is a nasty thing to do, but necessary. Cavity exams are gross, but necessary. So, taking organs from a heart-beating person because your authority told you to do so can blend into this routine. Third, once you’ve participated in something unconscionable, it’s very difficult to admit you were wrong. Especially when admitting this might mean losing your job. Upton Sinclair was right when he said “It is difficult to get a man to understand something when his salary depends on his not understanding it.” Nevertheless, more doctors are emerging who are willing to admit that “brain dead” organ donors are not dead at the time their organs are removed. I personally know two transplant surgeons who have changed specialties because they came to realize that the donors are not dead. There was a time when the number of doctors speaking out against brain death could be counted on one finger: Dr. Paul Byrne. Now, more and more doctors and organizations are questioning the brain death fallacy. I will be speaking on the ethics of transplant medicine at the 83rd Annual Meeting of the Association of American Physicians and Surgeons later this month.
