That is not all. Many insurance companies require a you to be on staff at a local hospital in state, have a state DEA number ( which is federal but you need a separate DEA for each state). This can take months to get credentialed. It took me about a year to get on my latest insurance.

Mark just look at code 66984. In around 1989 it was reimbursing close to $2000 by CMS. The surgery took about 40 minutes. Next year, 2026, medicare will cut payment to $460. It is a 10 minute surgery today. When compensating for inflation and the change in time it takes to perform the surgery, the reimbursement rate should be close to $1300. On top of this, we had no EMR to pay for, fewer and less expensive devices, less staff. This is just one of many examples of real numbers and how independent doctors have been squeezed.

Is food a right? We require it to live. The government does have a safety net for those that cannot afford it, just like heating, water, etc. But we don’t govern the whole industry the way healthcare is governed. Kicking the can down the road with renewal of Obamacare subsidies will not fix the problem.

Let’s start with Ohio’s real problems, because the state’s healthcare mess didn’t fall out of the sky, it was built, brick by bureaucratic brick. Ohio’s premiums aren’t high because Congress might end a temporary COVID subsidy. They’re high because Ohio, like Michigan, has created the perfect habitat for healthcare monopolies, and those monopolies behave exactly like you’d expect when nobody regulates them and everybody fears them. Ohio’s biggest hospital systems have more market power than the people who supposedly represent us. They negotiate their commercial prices in total secrecy. They eliminate independent physician competition by buying every practice within driving distance. They run tax-exempt empires while offering charity care that wouldn’t impress a bake sale committee. They siphon money from every federal subsidy program they can reach, 340B, GME, Medicaid DSH, and somehow never lower a single bill. And while families get crushed with premiums, executives stroll off with multi- million dollar compensation packages funded by “nonprofit” halos and state level dealmaking. That’s the real cost driver in Ohio. Not the end of a pandemic-era add-on. Not this bill. Not this vote. When someone claims “Republicans are taking healthcare away,” what they’re really doing is giving cover to the institutions actually draining Ohioans dry, the ones with the lobbyists, the tax deals, the subsidies, and the absolute refusal to compete on price. If we’re going to talk about healthcare costs in Ohio, let’s at least acknowledge the villain that’s standing in plain sight. You could fight the real fight…

A spending bill that takes health care away from millions and raises costs for 500,000 Ohioans is wrong, plain and simple. So I am voting NO on this terrible Republican bill.

These cuts are happening across the board. CPT Code 99284, standard cataract procedure, is receiving a 11% cut. I am not sure how much longer before independent physicians stop taking insurance and start taking cash only. It seems this is what the government wants to happen. They are creating a 2 tiered healthcare system.

I enjoyed the discussion. I think a subject that is almost never discussed is the ever decreasing payments by both medicare and commercial insurance companies to the physicians. It has cause doctors across the board, primary and specialty, to close their doors and sell to either private equity or hospitals. This is also driving up the cost of healthcare. Individual Doctors will continue to close their doors until there is a permanent payment fix.

@DrOzCMS I fail to understand how cutting physician payments every year will improve seniors access to high-quality care. The independent physician is slowly being wiped out and being replaced with large corporate hospital systems which end up costing more!

@DGlaucomflecken Would agree, however a close second is CMS reimbursement reform. Cutting physician reimbursement every year is truly damaging healthcare and a large reason private equity was ever able to enter the business in the first place.

@mikeduncan Don’t disagree with you Mike but shouldn’t we also ask, why is the cost of education so high today? It has spiraled out of control and our solution is to just forgive the debt but not to actually fix the problem. We need to look at why colleges charge so much.
