The large self funded plans need to stop using the large payer-PBMs and move to a transparent TPA and get the drugs through Cost Plus. Hopefully Congress will pass meaningful PBM reform as well.

PBMs are forcing predatory Part D contracts on Indy pharmacies. These contracts are draconian and unethical. If HHS and CMS choose not to intervene and fix this practice, they and their administrators should be held accountable for the damage to both businesses, patients as well

@ExpressScripts @ExpressRxHelp I have been trying to talk to a pharmacist for an hour I have been on hold. I am going to run out of medication that you’ve been processing for 11 days. Super awesome.
But because Optum doesn't handle the appeals, they can't touch the ticket and don't have ability to resolve the issue. So back over to UHC. The representative says that he needs more time to review the documentation on my account. I literally bust up laughing and cannot stop.
I call 96 business hours later (giving them a little benefit of the doubt that even though they say they're working on Saturday, I know they don't). This time I'm only bounced between 2 representatives. The Optum Prior Auth rep tried to run the claim - rejected.
24 hours later, I call and am told, oopsie, it's really 72 hours to wait. But I do get the representative to confirm that a ticket actually exists in their system. That's rare at this point in the game (Ask me how many times I've done this EXACT thing before).
Because UHC handles appeals, but their pharmacy, Optum, handles prior authorizations, no one really knows what to do to get my Rx filled at this point. So someone opens an urgent 24 hour ticket to link the appeal to the original request and Rx.
Now I call the pharmacy to fill the new generic Rx. But the pharmacy claim is rejected. After 2+ hours and several representative and department bounces, we determine that the appeal was never linked to the original prescription prior authorization.
I monitor this process closely because I'm so familiar with the points where it might fail. And I'm please when 24 hours later UHC approves the appeal. So now I have a prior authorization that was approved on appeal (that's important later, when everything goes to shit).
So my Gi's office sends a prior auth for generic Humira (adalimumab) to UHC. UHC rejects it. Who knows why. No reason is given. But within 24 hours or less, my dynamite GI's PA sends in another prior auth, worded slightly differently. Guess what? Rejected again.
