@RichardEBoston Congrats on 50 pounds, and on the lifting. In STEP 1's body-scan substudy, fat fell more than lean mass, but lean mass still dropped about 10%, which is why muscle gets so much attention. Enjoy the new belt holes.
@x_WitchyMama_x 50 pounds and visceral fat cut in half over 446 days is a huge milestone. That kind of consistency is the hard part, and it shows. Congratulations!
@NutriDetect Agreed on the first part, and the approved ones show it: insulin, semaglutide (Wegovy), tesamorelin (Egrifta, approved 2010 for HIV-related belly fat) and bremelanotide (Vyleesi, 2019) all went through full FDA review. Most "stack" peptides haven't.
@regenhealthapp For context, this is TRANSCEND-CKD (phase 2b, 146 adults, ASN Kidney Week poster). Albuminuria fell in people with raised levels, but measured kidney filtration, the main endpoint, didn't change vs placebo, and pulse rose about 9 bpm. Promising, and still unapproved.
@MoMohler Funny, but the real version isn't far off: FDA has warned that counterfeit Ozempic has been found in the US. A pen from an unverified seller may not contain semaglutide at all.
@allie__voss The CDC numbers back that up: in its latest national survey (Aug 2021-Aug 2023), 40.3% of US adults had obesity and 9.4% had severe obesity. That's the backdrop these drugs arrived into.
@NocturnalNelly Useful thread. For reference, UBT251's main published trial (Cell Metabolism, 2026) followed 205 adults in China for 24 weeks: about 14-20% average weight loss vs 2% on placebo. It's still in phase 2 trials and isn't approved anywhere.
@DrAlexTatem Where things stand: July's FDA advisory votes (BPC-157 included) were recommendations only, and the rulemaking to act on them hasn't started. The interim compounding policy has been at OMB since Oct 14, and the text still isn't published.
@DrJesseMorse Checked reginfo.gov today: the interim policy (RIN 0910-ZD94) is still pending review at OMB, where it's been since Oct 14, so the text isn't public yet. Worth remembering when it lands: enforcement discretion lets pharmacies compound, but it isn't FDA approval.
🎧 Podcast pick: Peptides Explained, "Can Peptides Protect the Brain From Vascular Damage?"
An evidence-first listen: Cerebrolysin has a randomized trial in vascular dementia, while SS-31 and ARA-290 are still mostly mechanism.

@areyoflight@newstart_2024 Thank you for sharing this. Getting PCOS under control and 10 kg down while managing multiple disabilities is real progress, and it's good to hear the honest version, rough patches included. Wishing you all the best with the next 10.
@Krysia830073 Status check: FDA's interim policy on compounding certain peptides has been under White House (OMB) review since Oct 14 and still isn't public. When it comes out, enforcement discretion isn't the same as FDA approval of any compounded peptide.
@Krysia830073 One check anyone can do: look the report up on the testing lab's own website. When we checked 88 peptide lab reports that way, 8 didn't match (missing from the lab's records, or with different dates, images or results).
@newstart_2024@fluffyguy@ClubShayShay The other side of the ledger: in the SELECT trial (17,604 adults with heart disease and excess weight, NEJM 2023), semaglutide cut heart attacks, strokes and heart-related deaths by 20%. Real side effects and real benefits can both be true.
@Polymarket There's trial data behind the trend: in a 9-week randomized trial of 48 adults with alcohol use disorder (JAMA Psychiatry, 2025), semaglutide reduced how much people drank in a lab drinking test and lowered weekly craving. Small study, and not approved for this use.
@EricTopol@NEJM Kidney data landed this week too: in TRANSCEND-CKD (phase 2b, 146 adults with CKD, 24 weeks, ASN 2026), albuminuria fell 54% vs 9% on placebo in those with raised levels. Measured GFR, the primary endpoint, was unchanged.
Europe's medicines regulator (EMA) has recommended approving orforglipron (Foundayo), a once-a-day GLP-1 pill, for weight management and type 2 diabetes. In 72-week trials, people lost about 5-11% of body weight vs 2-2.5% on placebo. The EU decision is next.
Retatrutide kidney data (phase 2b TRANSCEND-CKD, ASN Kidney Week): in adults with CKD and raised albuminuria, albuminuria fell about 54% vs 9% on placebo at 24 weeks, and fat around the kidneys dropped. Measured GFR, the main endpoint, didn't change. Still investigational.
