I grant that the website notes this data is old, but even if we generously say 30% make it to 24 wks, the rate is 27% survival. Which of course doesn’t account for the morbidity (and COSTS - both financial and psychological) associated with such prematurity.

The lines above this sentence on the website you pulled it from note that 50% deliver within 24-48 hrs of PPROM, while 90% deliver within 7 days. So that 90% is actually 90% of the 10% who make it to 24 wk

@3rahsin @RxHausted @islagiatt4 @abbeysuekos I also think you may be ascribing the failures of specific people to an entire profession, and that’s simply not fair. “Dr. Death” is a truly despicable person, for example, but we shouldn’t lose respect for all spinal surgeons because of him. (I am not a surgeon, to be clear)

@3rahsin @RxHausted @islagiatt4 @abbeysuekos I think we disagree on what their place is. I consider them an integral and necessary part of the medical team (with the same likelihood of being bad at their job as any other medical professional). You seem to think they should just do as they’re told.

@3rahsin @RxHausted @islagiatt4 @abbeysuekos A pharmacist is not going to refuse you a medically indicated, safe (and proven + FDA approved) med prescribed appropriately (dose, duration, quantity, etc). (Again, this excludes religious/conscientious refusals, which are protected by law, though I disagree with them).

I work in a hospital, so I may not be back on service for up to two weeks. Usually my staff can find me in that time, but not always. That’s an extreme, but it’s illustrative.

@3rahsin @RxHausted @islagiatt4 @abbeysuekos With the exception of religious refusals (which I personally disagree with but are protected by law in many areas), pharmacists aren’t refusing meds because they want to. That would impact their reimbursement. It makes no sense. They’re refusing for your safety.

@TinyTerrorsMom @RxHausted @abbeysuekos Ordering linezolid for a patient also on an SSRI. Serious contraindication, requires a washout period, and can be easily missed (eg, if a psychiatrist wrote the SSRI and the PCP doesn’t know - which happens all the time.)

(And at my school, the weighting of pharm questions on exams made it so that it was better for my grade to skip learning those questions in order to study other material. I learned for board exams later, but still)

@3rahsin @RxHausted @islagiatt4 @abbeysuekos Um, hi, I *did* get into and finish medical school, but I still rely on my pharmacist friends to help me out. I work in a hospital and am fortunate to have my unit pharmacist next door to my office. She is likely tired of my 300 questions a day. (I’ve been in practice 9 yrs)
