A Giant Korean Study of 15 Million People Just Blew a Hole in the “Lower LDL Is Always Better” Story
Imagine tracking almost 15 million adults for nearly nine years and counting every death from heart disease and stroke. That’s exactly what Korean researchers did with 14.9 million people who weren’t even taking cholesterol drugs. What they found doesn’t match the simple message that “the lower your LDL, the safer you are.” Instead, the risk formed a U-shape. People with very low LDL died more often from cardiovascular causes, people with very high LDL also died more often, and the sweet spot sat right in the middle.
For overall cardiovascular death the lowest risk landed between about 90 and 149 mg/dL. Below that range or well above it, deaths went up. The pattern changed depending on the exact problem. For heart attacks and blocked arteries the best zone was lower (around 70–114 mg/dL). For some types of brain bleeds the risk kept falling only when LDL stayed higher. In plain terms, driving LDL as low as possible protected against one kind of death while raising the risk of others.
When the scientists looked only at the higher range (100 to 300 mg/dL) they still saw a clear rise in heart-attack deaths as LDL climbed. Each jump of about 40 mg/dL raised the chance of dying from ischemic heart disease by roughly 26 percent. But that rise was weaker in women and in older adults than in younger men. Even people in their late 70s and early 80s still showed some extra risk from higher LDL for heart attacks and ischemic strokes, yet the overall picture never became “lower is always safer.”
The study is huge, it followed real people instead of just guessing from lab tests, and it only looked at folks who were not already on statins. That makes the U-shape harder to wave away. Doctors still treat high LDL when people already have heart disease or several risk factors, because the top end of the curve is real. But the data also show that pushing every healthy adult toward rock-bottom LDL numbers is not free. Too low carried its own price in this massive population.
The takeaway is simple and uncomfortable: LDL cholesterol is not a one-way street. In nearly 15 million Koreans the safest zone for dying of cardiovascular disease was not the lowest number on the lab report. Both extremes cost lives. That single finding from one of the largest studies ever done keeps forcing the same awkward question—how low is actually low enough, and for whom?

Your brain needs energy, not a steady delivery of pasta and bagels. Your body has backup systems that can make glucose and provide alternative fuels when carbs are limited or restricted.
People who eat low carb/high fat know this.
If you haven't ever had ketones fueling your brain, you cannot grasp the full meaning of the words 'mental clarity.'
"The brain must have glucose!" That's what we've all been taught. Our new study is the first evidence suggesting it may not.
We kept slices of mouse brain alive in fluid with no glucose and only the ketone BHB as fuel. After 4 hours, the mitochondria were still producing energy,

Coming home from the Ketogenic Foundation’s conference, I can’t help but feel optimistic about the future of metabolic health and our ability to treat debilitating conditions. So many great conversations that I otherwise would never have had with the preeminent researchers and clincians in the ketogenic metabolic therapy world.
My wife @LauraBuchananMD and I were medical students in 2016 when we listened to a podcast with two crazy doctors, @DoctorTro and @BrianLenzkes, that were helping patients come off hundreds of units of insulin and achieve type 2 diabetes remission. A podcast with Sarah Hallberg reinforced what we were hearing. These were foundational to our practice of medicine.
Our medical education hadn’t prepared us for those conversations. We found them by chance.
That’s why I’m especially encouraged by the Ketogenic Foundation’s work developing medical education. The @TheSMHP also fills a role with conferences, grand rounds, and journal clubs. Together, they fill the gaps missing in 2017.
Future physicians shouldn’t have to stumble across the right podcast to discover ketogenic therapies and metabolic medicine. Their training should introduce them to the evidence and its clinical potential.
Beyond these metabolic improvements, the conference focused on emerging data of remission in severe, treatment-resistant mental illness. We’re also hearing about patients with glioblastoma surviving far longer than expected (and some are still alive!) in addition to a dozen other conditions.
It’s been a decade since those first podcasts. I began practicing this approach in residency and have spent the past three years honing it as an attending. What once seemed extraordinary has become part of my daily clinical work.
What excites me most is what this could mean for patients: fewer medications, greater independence, and an improved quality of living that would be unheard of just a few years ago.
The momentum is building. The inertia is there. But patients are suffering now, and we have to keep pushing this work forward. 💪
It's not too late to join us!

Matthew Calkins, MD, MHP, DABOM, presents:
A Critical Appraisal of Iatan et al.: "Association of a Low-Carbohydrate High-Fat Diet With Plasma Lipid Levels and Cardiovascular Risk."
Just published:
Carbohydrate reduction for metabolic disease is distinct from the ketogenic diet for epilepsy
Background: @AmerAcadPeds recently released a paper on recommendations regarding low carbohydrate diets in children with T1D, T2D, or at risk of T2D.
Problem: They conflated 3:1 or 4:1 ketogenic diets classically used to treat epilepsy with well formulated ketogenic diets that are used to treat metabolic disease.
The AAP paper reported or recommended
1.) Checking 14 different laboratory measurements via 5 separate blood draws within the first year of starting a KD, including monitoring carnitine levels and a DEXA scan at year 2
2.) Caution that low carbohydrate diets cause eating disorders when there is a lack of evidence that any clinician-led eating pattern changes causes eating disorders
In reality, a well formulated KD based on a modified Atkins Diet fulfills the AAP's recommendation of 5 servings of vegetables per day. A WFKD exceeds all of the minimum nutrient reference values for children and adults.
We agree with the AAP that all eating patterns require more research in pediatric patients. In order to optimize this research, common definitions and terminology must be used. Our paper calls for future reports to enlist the assistance of clinicians who utilize therapeutic carbohydrate reduction in their practice.

@AlanaNewmanCA Glad to be hear!
Such an important topic. 70% of the calories our children consume comes from ultra processed food, and 50% of the calories pregnant women and postpartum women consume is ultraprocessed
